- Patients may be seen with or without a referral to our office, depending on the individual’s insurance. We participate with most insurances, some of which do require a referral from the primary care physician.
- As a professional courtesy, we will provide shots to patients whose serum is from another office. However, patients whose serum is not made in our office will require an office visit with one of our physicians in order to receive shots with us.
- It is the policy of this office that the adult presenting the child for treatment is responsible for payment of the patient portion at the time of service.
- Please notify us as soon as possible of address and insurance changes so that we may contact you in a timely fashion, and bill your insurance plan appropriately.
- Food and Drinks are not allowed in our office.
- Use of cell phones to make/receive calls is restricted to the entryway of our office building. Please exercise respect for our patients and staff by keeping cell phone use limited to this area.
- We ask that you refrain from using perfumes, colognes, and strongly scented lotions or other products for the benefit of other patients who may suffer increased respiratory symptoms with exposure to these products.
