Prescription Refills/Contact a Nurse

When contacting the nurse, be sure to include:

  • Patient's first and last name, with complete spelling
  • Patient's birth date A phone number where you can be reached
  • Be sure to also include the nature of your problem, such as difficulty controlling symptoms, or feeling ill.


**If you are running a fever or experiencing vomiting or diarrhea, we ask that you contact your primary care provider for treatment.

**Please be aware that your concerns will be addressed during normal office hours.

**If you are having a shot reaction or other urgent problem during office hours, please call the office directly (616)531-6900.

**If the office is closed and you are experiencing a life threatening emergency, call 911. If you require after hours assistance for other medical concerns, please contact the after-hours service through our main office number.

When leaving prescription refill requests, be sure to include:

  • Patient's first and last name, with complete spelling
  • Patient's birth date
  • Name of medication to be refilled
  • Name of pharmacy where prescription is to be filled
  • A phone number where you can be reached if there are questions regarding your request

Please allow 24 to 48 hours  for your request to be processed.
